SMARTVISION PRODUCTS ORDER FORM

Prior to placing your product order, please make sure you read the section on Eye Dominance. Then select the appropriate SmartVision glasses to order, right or left eye dominant. We also offer you the option of ordering all product packages with two pairs of glasses, one right and one left eye dominant, for a discounted add-on fee.

Please make your product selections and then indicate quantities in the shopping cart. SUBTOTALS for each package appear below. TAX and SHIPPING FEE per USPS will appear in the shopping cart. A $7 shipping and handling fee is added to all packages, based on priority mail within the United States. If you are ordering from outside of the U.S., we will notify you by email of the additional shipping fee.
